CRAFT VENDOR REGISTRATION

                  •        Please reserve a space for me to display and sell my crafts during  “SHRINE FEST 2010” @ The Oleika Temple’s Great Lawn in Lexington, Kentucky July 24^th, 2010.

                  •        Space rental is $100. Each space can fit a 10 X 10 tent.

                  •        Mail Registration Form along with a check or money order to:

                  •        Please make sure your check or money order is made out to OLEIKA TEMPLE

                  •        All Entries must be received by June 15^th, 2010

         •   Please call David Hadland @ 917-865-6977 to confirm that space is available before you send in a fee, as the spaces are limited.

Assumption of Risk: By submitting this registration form and signing it, the artisan (and each individual member thereof) assumes all risks of property either lost, stolen or damaged or bodily injury (including death) that might occur during the set-up and/or the festival. The artisan (and each individual member thereof) agrees not to bring suit against The Oleika Temple and its members on any claim for property lost, stolen, or damaged or bodily injury that might occur during the festival.

Information:

                  •        The Spaces will be (insert space size here, ie.12 x 12 ft.)

                  •        Electric will be available for the vendors. Vendors must bring a 50 ft 3 prong extension cable.

                  •        You must have your own tent, covering, canopy, if you desire and your own tables. Oleika Temple will not be providing them.

Vendor Fee:

                  •        $100 space fee includes festival admission for 2 persons.

Vendor Set-up:

                  •        Vendors can set up there spaces starting at 8 am.

Vendor breakdown:

                  •        Vendors must be off the premises by 11 pm. The festival ends at 9 pm.
